Inugami Akira from Wolf Guy - Ookami no Monshou, he´s definitely hot.
I also thought maybe Kurono Kei, from Gantz, but actually, he´s probably cute more than hot or handsome, especially when compared to side character vampire Host Samurai.
I guess that mangakas usually make the hero average so that guys can relate to him and create a hot secondary character to attract girls as well.

I tried to read the seinen Jiraishin because the main character was hot, but it was too episodic for my tastes.
Tenma in Monster is pretty hot, when he's not all five-o'clock-shadowy.
You can also put me on the Reborn! fangirl list, although the main character Tsuna is more cute than hot, but the other guys are smokin'.
And of course Saiyuki, a fangirl's dream.

I know he's not THE main character, but Rukawa from Slam Dunk is pretty hot. There's some other hottie basketball players mixed in.
