King of Thorn ~Survival Manga.
Omae ga Sekai wo Kowashitai nara ~Vampire-like manga.

Arcana ~"At times you'll find it confusing" manga. XD
Doubt (TONOGAI Yoshiki) ~Another survival-game manga...

Dorohedoro ~Bio-punk world, magic users, mystery, comedic killings.
Dokuhime ~Slight "time skip" & poison Princesses.

If you haven't read it yet, Death Note.

Berserk has a major interesting plot twist too.

20th Century Boys is riddled with plot twists.

Definitely second Vampire Juujikai and anything by SHIRODAIRA Kyou.

I would also recommend Hoshi no Samidare due to the just randomness of the manga.

On the randomness track, FLCL is also a WTF manga. The story is quite convoluted, and requires much rereading to understand, or rather suspend disbelief enough to believe.

But I'm guessing you don't really want a WTF moment, but more like a coherent surprise. What about Utahime, where what you think is supposed to happen doesn't really happen. Aki-sensei is one of my personal favorite authors (her art is so pretty), and you might like some of her stuff.

Amatsuki also has a lot of "wow, didn't see that one coming" moments as well. The story basically revolves around a young boy who gets whisked into a virtual reality that becomes real, but he has a special power within that world. My summary does it no justice.

You could also try this novel and anime movie of the same name called Paprika. The villain in particular was rather unexpected, and it also has a lot of WTF moments due to its content: dreams, and the manifestation thereof.

I ESPECIALLY recommend Classmate, Kamimura Yuuka wa Kou Itta.. It may start out rather WTF at first, and it may seem like a Haruhi clone (at least until the second chapter), but keep with it and it's actually quite mindbending. It seems to delve into two philosophical theories: Philosophical Zombies and Simulation hypothesis. The time the girl showed what exactly the world was, it's was pretty unexpected.

Don't quote me on this, but I think Shounen Hakaryuudo has some "never saw that coming" moments. I haven't read it in a while, so I can't say for sure, but I do remember that the manga had amazing art.

I mean I can see pretty much every twist and turn coming there are only very rare occasions when this is not true but I guess that's the price I pay for being a writer.

Melo Holic Just finished moments ago and it had AMAZING twists seriously a great comic.

like god says - survivial manga with maaaaaaaaaaaany shocking twists.

cradle of monsters - again survivial- but enormous twists start apearing after about 15 or 20 chapters or so...... though it's startling as it is at begining without the big twists too =^^=.

i'm only telling the most shocking twists.... leaving aside the huge amount of run of the mill traditional ones in most of the series.
