i havent read the whole thing but from what i have read and at the point that i read, the romantic interest knows she's a girl, and if I remember correctly everyone else that lives in the house with them knows it too. the "love-rival" doesnt know shes a girl and sometimes gets peeved that he pays so much attention to a boy but brushes it off saying that she has nothing to worry about bc its a boy. i havent read far so i dont know if there are any other twists that happen
The love rival will also know that she's a girl.
He meets a girl somewhere( I don't remember where -.-) and he keeps thinking that it would be nice if that girl and the heroine were the same person. So he's really happy when he finds out that they're the same person. (I might be wrong, I haven't read this manga for a long time)

Sorry, don't really have anything to contribute. But just wanted to say, if you haven't read Tokyo Crazy Paradise, do!! Even though the main lead guy knows the gender of the girl, it's pretty awesome! The girl is really cool, and kicked serious ass. PLUS, it's absolutely hilarious and adorable.
I've read it three times now, and really, you won't regret it!

One of the love interests knows she's a girl, but the main (so far seemingly main?) romance thinks she's a boy for quite a while. However, he doesn't question his sexuality at all -- he's fine with it either way, I guess he's bisexual.
