I'm looking for manga that the main character has a disorder, such as alice in wonderland syndrome, or so forth. If it is possible that NOT romance or ecchi. Is there a recommendation 😕

I Am a Hero's protagonist experiences hallucinations, though they lessen as the series progresses. Very minor romance for most of the series.

Usotsuki Mi-kun to Kowareta Ma-chan - Totteoki no Uso

Shi ni Itaru Yamai

The Friendly Winter

EDIT:
If you want to see metaphorically distorted art based on the psychological stress of the characters, Life is Money is a visual treat. The protagonists don't have disorders, but it's cool enough to mention since it's in the same vein.

Himizu and Nijigahara Holograph, but they both have romantic subplots.
Homunculus, in a way.
Koroshiya Ichi -- This is very violent, though.
Tajuujinkaku Tantei Psycho
