this manga is about an uke and seme who were childhood friends.
The seme always used to follow the uke around because the uke was his protector.
The seme moves away to some foreign country, and fast forwarding years later, the seme is a really cool, playboy man now.

I think one of the characters was named kei or karen.
The art is kinda old but not too old. ( I don't know how to describe it)

I first thought of Kimitachi Otokonoko, but that's probably not what you're looking for, I guess? The seme was never protected by the uke there. Maybe Aitsu no Daihonmei?

awww, that's not it either..the art looked a bit older, but not to old, just along makota tateno's art work.
This manga might have been a one shot, or a part of a collection of stories in one volume.

The only other one that I can think of that is kind of fit is Demain Syndrome but it's the uke that moves to a foreign country.
