Elfen Lied's violent, but its certainly not the most violent. Its little more than loads of blood splatters, wheras other mangas have much more graphic, detailed violence.

Ichi the Killer is one of the most violent I have read. I would have to put Berserk up there though for the insane monster rapes and crazy torture devices.
