I could think of some really complex deep mangas that I barely couldn't understand half of the plot O__o;;
I love Kaori Yuki's works but sometimes, it's too deep...!
Shin Angyo Onshi. << complex in mid-plot. but it's overall good.
As for other mangas that is hard to get an idea on what the h*ll the mangaka is saying (in a bad way) is.... Hot Blooded woman

I was so confused after I finished Camelot Garden. Kaori Yuki's works are hard to follow sometimes.
Tsubasa RC gets pretty confusing too. Too many plot twists. >_<

Yeah I thought the same thing. And I've never "fallowed" a manga before either...
I'd probably say the most hard to follow manga I've ever read is Tenjou Tenge

Seconded. Art was good, but it was one series that was confusing as hell.
I'm going to add Nijigahara Holograph. I've read it and re-read it a while back and was confused as hell after each time, despite it only being one volume.

How was BLAME! confusing. All you had to do was watch the next panel. There was hardly even any speech bubbles to get in the way.
Agharta is bloody hard to follow. FLCL although its only two volumes, is the definition of spazzy.
Sarcasm just doesn't work over the internet.
Vampire Knight - Yeah I was just lost after chapter 40.
Blame! - Yah that just kinda made me do a doubletake...

Phantom Wizard lost me straight away and was by far the most confusing thing I've ever read. The other manga's I found took a couple of reads to fully understand are.
Angel Sanctuary ~ Love it but too many characters first time round!
07 Ghost ~ Terminology confused the Hell out of me first time
Diabolo ~ Great but the first volume has too much information and jumps around like crazy until you have no idea what is going on.
Nijigahara Holograph ~ Good but requires one sitting to avoid getting lost.
The pen is mightier than the sword...and considerably easier to write with.
Oh how could I forget Tsubasa Reservoir Chronicles? The latest 50 chapters or so make my brain go on overdrive...
