I've been reading a lot of reincarnation stories and I have yet to come across one where the fact that the MC is reincarnated has be revealed. Now many of the stories are not finished, so it is possible that it could happen in them and I just don't know that yet. But does anyone know a story where at some point the truth is revealed?

I suggest using spoiler tags, as I would guess that something like this would be a major spoiler.

There's Tensei Shitara Slime Datta Ken (Novel) . They kind of gloss over it though but it might play a major part in the future

mushoku tensei.
