How do people translate names into Japanese?

For example: 菅原明日香 is Sugahara Asuka
For those that uses hiragana or katakana in their names, it is usually changed into its closest kanji equivalent.

I have yet to see a takubon sell for less than 700 Yen in the past 3 years and I do go to Japan once a year. That's about $6 US.
