With the twin brother appearing only in flashbacks: Mars, Tsuki ni Kiss no Hanataba wo.
There's also a twin brother issue later in the series of Hana Yori Dango.

A lot of good recommendations up here. Personally, I find manga unique when it deeply touches my emotions and disturbs my thoughts.
