Homunculus - In this case, it's more like the characters are really bizarre and abnormal people - but I don't know if they have any actual mental illnesses...

If you consider sadism, masochism and psychopathic tendencies as mental disorders (which they are, of course); Koroshiya Ichi should fit too. But be warned - it is extremely gruesome and graphic.
