I think there's a few stories in Petshop of Horrors were the owner falls in love with their pet, there's also the 2nd story of Shin Petshop of Horrors but the animals are in animal form for nearly none of the time in these.

Gakuen Sousei Nekoten!, though the romance isn't really official. And also, Kaeru no Saiban.

Vampire Game sorta, only he was originally in human form (if vampire counts anyways). He spends a good chunk of the story in animal form though, and does some shape shifting...though he's not exactly the main love interest.

@imp4ever, I'd rather stick with a character that's originally (actually) an animal, but the manga sounds interesting anyway. Will add it to my to-read list!

To those that have recommended Tokage Ouji... The lizard was actually originally human, as is revealed quite quickly in the first volume! Just thought I should clarify that... Lots of fun, anyway.

Still on the lookout for more "true" animal/human romances. Thanks to those whose suggestions followed this requirement.

for human/real animal, I suggest Wild Cats, a lion has a crush on his master, very amusing and touching at times.

Children Can't Choose Their Parents, human/chicken, kind of twisted and disturbing for some people, but I lol-ed so hard at the ending.

Two titles above don't have animal turns into human, though.
