I remember seeing one time that it's from a novel, but the person who posted the picture never posted the name of it.
Your best bet is to do what zombiefetishlove recommended, and attempt to contact the person at the livejournal.
In the mean time, I'll try to see if I find the name anywhere...

I can't read Kanji, so the name Google translated is something along the lines of "Farthest Sea, Heaven's Door".
Sorry I couldn't help more.
Edit 2: Didn't notice the katakana on the page (how dumb of me), but the Google translation was very close to being right for once, so I'm confident saying that the name is what the translator gave me.
I also found the name of the author: It's Aiba Rin. His/Her page is not on this site, yet.
Had trouble with the artists name, but it's along the lines of Mizumi Fujiko? Not sure here again.
