Perfect Girl Evolution might fit, though it's also a "forced" make-over, and may take a while for the charachters to...umm well you have to read i to know!! 😀 lol

What about Othello (shoujo)?
The heroine has split personality but that's some kind of a change, right? Both her looks [not much though] and personality change 🙂

Later when she accepts her other self she changes and becomes the combination of both personalities.
