Didn't some series end with some girl taking an axe to the guy protagonist and cause some media uproar too?
Then again, most of those requirements would make for a crappy manga...or a very interesting 4-kon series...

I hate random moments of 'originality', simply for the sake of being 'different'.
A unique plot is great, but unpredictable events should fit seamlessly into the series.
The difference is in good storytelling.

There are some really good, totally unpredictable manga out there. Banana Fish is my personal fav but there are loads more.
