I got stuck in the original Kingdom Hearts 1 at the Hollow Bastion stage after trying to jump from one ledge to another that comes out of the wall, thinking that it's where i'm supposed to go. I could never make it. I did this for an hour until i walked into the door in front of me that triggered a boss fight.... That made me lol.

Pokemon Gold
I had just gotten it for Christmas after it came out and was playing for 14 hours straight when i was starting to get a headache so i turned the power off and i hadn't saved once!

Halo 2
Was playing capture the flag with a big group of friends and i was supposed to be guarding the flag but i spaced and didn't notice the flag was gone until they were halfway down the field. Then 5 min later after my friends beat me down i did it again!!

The guys are still laughing at me for that one and now when we play they say "don't pull a birdo"

Borderlands: I decided to go through the maps picking up the bad weapons and selling them, just on a whim. Left my game on when I ran an errand. Little brother decided to sell every piece of equipment I had, including my beloved Eridian BFG9000.

Fallout 2: Decided that I was the dominant species and slogged through a lake of radioactive goo. Little did I know that that mutates you.

Fallout 3: Decided that slavers were to be eliminated. Ran out of stimpacks, was forced to hide in a bathroom stall with a minigun.

Ninety Nine Nights: Didn't realize you could equip items, so I had a helluva time getting through the game.

Later, when I figured out the items thing and got some new characters, the game glitched up and saved every file I had as the same (just started) character.

Half Life 2: During like the first actual mission, I got stuck, not realizing the way to go was ridiculously obvious.

Just did one in Fallout 3 last night. I had fast traveled to Northwest Seneca Station, and there were a group of Regulators waiting for me (My character is Very Evil at the moment), I had saved it cause I was about to go to sleep, and when I woke up earlier to play some more, I saw that I saved it with just 2 HP left and with 3 Regulators right by me. Luckily I was able to get away, cause if I didn't then I would've been stuck in a death loop (And it seemed like I was, cause it took about 1 1/2 hours to successfully get away).

Dark Messiha Might and Magic, Chapter Five, Mountain-Side Path

I had 3 points in Melee Combat so i could charge my opponet (runing at them holding in the charged attack) and the orc stands at a Slight turn just at the Edge.
I Sprint forth prepare my charge and he doesn't have time to react, but I miss him with the first attack and end up on his left (BUT CHARGE CONSIST OF TO SWINGS) and I turn towards him and the second swing (wich i had forgoten about) comes and my charater steps forward as the orc reacts to my precense and does a doge like motion that I expected for the first attack.
Now however I miss him again and runs straight out the cliff and starts to fall,
So i qickly presses the Quick Save...... %¤#&!!!! Since then I've been keeping the Quick Save and Load buttons futher apart

When I first got Pokemon Red when it first came out I of course picked Charmander as my starter. Well unfortunately you have to fight brock then misty and charmander gets raped by both of those gym leaders. Well I zoned out when it showed how to catch other pokemon so ended up trying to beat them with only charmander. It took me until I got a charmeleon to finally beat brock and then to beat misty the only reason I won was because charmeleon learned slash in battle and I got a lucky crit that killed starmie. All together I probably wasted 20hrs trained up to almost a charizard only beatting lvl 3-5 pidgey.

Another time I was playing Diablo II. My friend only liked to play hardcore so every time I died which was once a week I had to start over. Well one time during the first act I happened upon a Gull Dagger with +100% to find magic items. My friend insisted that I trade him it or at least give him the item for safe keeping. My character lasted longer then all of the other ones before it but I still died without giving him the item and he was extremely pissed but i loled pretty hard.

I was playing FFX-2 and I suddenly had to use the bathroom while I was fighting a boss. I didn't realize that I'd forgotten to pause, and when I came back, I had game over.

while playing final fantasy 7 i was raising a gold chocobo after around 12 hours with partial progress, fell asleep and when i woke up the tv was off, apparently there was a power surge. didn't save.

something more recent while playing Mass Effect 2 for the first time in the final suicide mission of the game
Spoiler (highlight to view)
I left Mordin in hold the line quest and took Garrus and Legion to final boss battle, with all loyalties and ship critical upgrades done, mordin ended up dying. Mordin also died in my next two playhtoughs, coud not get the No one left behind achievement. Later realized that though characters like Garrus, Zaeed and Grunt ahve to be on hold the line and its best to send mordin on escort duty :P

One of the dumbest things I've ever done was accomplished while playing the first Golden Sun for GBA...
I had played through it almost to the end, but I got stuck in the Venus Lighthouse where you had to cross this one river of sand that was shorter in length and wider than any other in the game. I tried dozens of times to cross it, but I could never make it, and always fell off the ledge. So I handed it to a friend of mine, who immediately crossed it in one try. I took it back and tried myself a couple more times, but I still couldn't make it. So I asked my friend what I was doing wrong; he watched me try, and said, "Dude, why aren't you running across it?"
I was stunned. I had no clue that you could run. I had gone through the whole game... walking. I had leveled my people up to lv. 80; I had been roaming around for hours upon dozens of hours of playing time in this game, contentedly ignorant and walking the whole time. And I'm no newbie gamer either; I had played at least a dozen games before this where you press B to run, like in this one. I guess I just never thought about it, and for some reason I didn't end up doing it out of habit either.
So yup. Stu-pid. Can anyone top that?

You can run by pressing B!? eek
Wtf, someone please give me a facepalm reply cuz I deserve it >.<

In Fallout 3 I tried to kill a Super Mutant Overlord, and 2 Brutes with a pistol with 10 bullets and no stimpack... I didn't die (barely)... And of course I didn't kill them either... Buut while I was running like a crazy to escape I steped on a landmine and died...

I play Fallout like a espionage game... (this annoys my brother, because he plays it like a war game) So I'm always crouching... and once in a while I get sutck in places that if I shoot, or don't move, or run... I'll die...
