In the Clothes Named Fat its about the main character's obsession with her weight....the ambiance is so similar to requiem for a dream and im pretty sure you'll enjoy reading this if you liked requiem

Maybe Holyland? Several of the characters get addicted to drugs and drugs and those who sell them become an important part of the storyline.
