The most influential shounen romance and harem manga should be Urusei Yatsura. A lot of harems still follow its model and have the same ecchi situations.

I think shoujo mangaka tend to be more influential in terms of their overall careers than their specific series, which seem to be much shorter than standard shounen series. The series that springs to mind most immediately is Rose of Versailles: the influence of this thing on all later shoujo manga, Takarazuka, and all other entertainment for Japanese women is unbelievable! In terms of careers, I'm going to be boring and go with Hagio Moto and Takemiya Keiko (and the "24 Nen Gumi" aka "Fabulous 49ers" in general). Again, their influence really cannot be overstated.

Dragonball and Slam Dunk, easily.
Slam Dunk made basketball relevant in Japan and has kept it that way. There'S even Slam Dunk scholarship awards handed out.

But I wish most harem series could be anywhere near as awesome as the Urusei Yatsura movie. : D
As for most-influential series... I'd say Astro Boy. The work of the 24-gumi is the most influential in the shoujo sphere, especially Thomas no Shinzou and Shiroi Heya no Futari. The latter really isn't very good, but it was the first shoujo-ai and set a precedent for the cliches of today...

We are talking about manga not movie adaptations. They are not the same. You will still find the same tiring stuff/jokes in today's many harem mangas, but the latter are not even as good as Urusei Yatsura. BTW such harems suck. I don't really see the appeal no matter whether it's the first one or not.
