Paranoia Star. Brain-bending, though, be warned.

Ichi the Killer has a protagonist that isn't necessarily paranoid, but he is delusional - I mean, he has some pretty weird ideas about life and goes around killing people. >_>;

Homunculus has a protagonist that is apparently suffering from the effects of trepanation (a kind of bizarre head surgery) and therefore "sees things", but for all we know, they could be hallucinations/delusions...

Not really paranoid, but I think you'll understand once you read it.

And anything by Matsumoto Jiro would probably meet your criterion if you ask me... but who knows?

Freesia, and others by the same mangaka. Though some don't have characters afflicted with specific symptoms of schizophrenia, it certainly gives a feeling of being high on shrooms.
