Sounds like Kakumei no Hi
This manga is about a boy who is reality, genetically is actually a girl. Thus has surgery and becomes one.
As a girl she;
- Develops a somewhat fear of men. < (Actually, I'm not sure about that one. It's been awhile since I read the manga, but it's something along that line.)
- Has 3 gorgeous guy friends going after her.
- Ends up with her best female friend's lil bro.
