After reading a lot of mangas, it gets lame, as you get bored of the lack of original ideas and the fact that you don't really get shocked anymore, only slightly surprised.

I am looking for something with lots of twists, where the mangaka likes to take risks and where important characters die throughout the story. I also wouldnt mind the main character dying at some point and being replaced by someone else. My top requirement is that it has good art.

And anything too "fluffy", if you get what I mean.

As for genre, I'm open to anything that doesn't involve Sci-Fi, Mecha, Loli/Shotacon, Gay stuff, shoujo-ai and shonen-ai.

Saver is everything you want, and more. Intricate plot, intelligent ideas, LOADS of character death (even of major characters), awesome world-building.

The art's Korean-style, though, so it might take some getting used to. All the characters are ridiculously good looking, but I wouldn't call the art fluffy, by any means - it's more angular and sharp than anything else. There are no flower tones or sparkly bits, for one thing. Lol.

Anyway, I hope you enjoy it.

Another manhwa I'd recommend is Dangu. Excellent stuff, also has major characters dying (even the main character dying and being replaced).

For shock value, if you're looking for something that'll freak you out and surprise you endlessly, consider Burning Hell. Craziest protagonists I've seen in a long time.

ZeroIn got some twist afair, and one HUGE wtf moment (around halfway through the scanlations) - it might be what you're looking for but then again, there's only very few stories that are completely unpredictable... it's worth a try at the least.

How about Gantz? (Although it does have a Sci-fi theme, it actually does the series good) Lots of massacres going on sometimes eek
And I recommend Mirai Nikki - hope you haven't read that one already ^^

+ all Deep Love series - tragedy, sad, all kinds of shocking concepts in this.
+ Doubt (Tonogai Yoshiki) - game where a group have to find the killer whilst being killed off one by one.
+ Goth - a boy with a dark fetish
+ Majin Tantei Nougami Neuro
+ X-Day - students want to blow up their school

Well, Naoki Urasawa work describes what you want. Especially the 20th century boys. http://www.mangaupdates.com/series.html?id=319. About the art, if you are not used to it you will think that's it's bad. But after a while you will find that the characters expression is actually really good and of course the art as well.
