I understand what you mean, but I don't really understand how a "random manga" button with some settings so it won't suggest certain genres is any different from just doing a series genre search, excluding certain genres and then just picking one from the list at random. ...

Not a manga, and maybe you've played it already, but the visual novel Katawa Shoujo probably has a route or two that fits your requirements. All the love interests have a disability of some sort, while the protagonist has a heart condition. It's very good.

Found it on DA. The artist calls it "Foxy Roxy" - might be an original character, or possibly a fanart... have a poke around the artist's profile.
Edit: oh hey, just noticed the artist description says character is copyright of Gametek, so... it's from something. ...
