Miriam - very sweet manga set in the Wild West. The hero starts out 17 and the (awesome!) heroine is 8, and then there's a second section when the girl's grown up. Definitely recommended.

Umm... I would say Immortal Rain. She's 14 and he... appears to be 24. I'm not sure if this really counts. He met her parents before she was born.
Also, if you're looking for shoujo, you might want to try Yumemiru Taiyou. It's surprisingly refreshing and adorable. The characters are currently at what could be seen as "acceptable", but this also might not fit your requirements because they didn't meet when they were younger.

This. When I read the thread title, I thought "Oh, s/he's read Miriam and wants another story like it." ...that's how perfectly it fits the request, lol.

^generally many think the ending is great because it fits... she made the ending fit, as if it's not axed, even for an axed manga, cuz usually when a manga is axed they didn't give a good ending (ex: to love ru) and she let it open... she ended the story by giving that point where the reader start imagining how the future is like...
