I only rarely watch anime based in manga series... I've been dissapointed too many times, there are very few that managed to show respect for their source material.

Best:

Death Note
Was more or less faithfull to the manga, not many complains here.

Inuyasha (Season 1-2)
Despite the sometimes shitty animation these seasons were pretty awesome.

Mermaid's Forest
Pretty neat anime, that followed the manga pretty close. Too bad they left out one of the stories.

Worst

Gravitation
This anime is the worst piece of dung I've ever seen. It's like they took all the stuff that made the manga awesome, and removed it. None of the characters act like they do in the manga, the humor is completely gone, it only loosely follows half the books and therefore they had to make up their own crappy boring ending, and last but not least... The animation is downright terrible, some of the worst I've ever seen

Chrono Crusade
I love the manga too much to be able to accep what they did to the ending... I also hate the voice casting for the main characters, and I think the character design was wierd too, didn't look anything like the manga.

Princess Princess
Speaking of wierd character designs... I kinda get the feeling that whoever did the character design for this anime, did not look at a single page of the manga. Also the anime's reaaaaaallly pushing the gay tones, which is annoying in a story that should be about friendship.

Ouran High School Host Club - Hands down one of the nicest manga-to-anime adpations I've seen. The stylish animation, the great voice acting, and most of all, balancing new plots with the original plots. It's difficult to stay faithful while being creative, and Ouran is definitely an anime that has done it excellently.

Ouran High School Host Club vibrant in it's voice acting as well as it's portrayal of the plot
Itazura na Kiss Love the anime because the art was much more in tune with modern art! For some reason I can't bring myself to like old art :/
Nodame Cantabile It had all the important plot details, the voice acting was impeccable and spot on, and the music was brought to life! biggrin
Mushi-shi Oh my goodness, the colors were so bold and vivid, the images beautiful and detailed and the music mysterious yet gentle which was also the quality of the voice acting *thumbs up*
Ghost Hunt scared the sh*t out of me much more than the manga, plus the art was much better

Worst
Chrno Crusade I stopped watching within the first 10 minutes because I could not stand the voice acting. Other reviews do not encourage me to pursue it further :/
I've managed to avoid most of the worst anime that everyone else has mentioned bigrazz

Fullmetal Alchemist, one of the few animes that actually improved on the manga.
Yu Yu Hakusho. Got a great voice cast for the English dub and improved on the ending. Great music too!
Elfen Lied, diverged from the manga, but still kept the message and the content intact. Didn't skimp on any of the violence, and in some ways improved on the manga's ending.

Worst
Fullmetal Alchemist: Brotherhood. Stayed truer to the manga, but lacked the heart and emotional connection of the first anime adaptation. Completely unnecessary to adapt the manga a second time to stay truer to the source material. The anime and the manga are two separate entities, each with their own strengths, so there's no reason to create a carbon copy of the manga in anime form that has nothing new to add to the series. May be truer to the manga in terms of story, but falls flat when it comes to the things that actually matter.
Dragon Ball may be a classic, but about a third of the series is filler that adds nothing new or interesting to the series. More tedious to watch than enjoyable, as the filler completely screws up the pacing of the series. That being said, the English dub is still fun to watch if you're a fan of so-bad-its good voice acting and ridiculously cheesy dialogue.
Berserk, not necessarily a bad adaptation, but at least finish the thing! I was hoping that after the Golden Age arc, they'd diverge from the manga and create their own story about what happens to Gutts and Griffith. I think it could be great if you took the characters and the backstory and did something totally new with them, while still staying true to the original, much like Elfen Lied or Fullmetal Alchemist. Hopefully the new Berserk series will deliver.
