The guy starts to work for a mafia/gang leader. If I'm right he is the bodyguard. He is impotent. The mafia leader who sleeps around with anyone takes interest and always tries to get the bodyguard 'up' who let's his boss do whatever he wants.

The bodyguard is impotent because he saw his sister being raped and used by his dad.

I can remember the details and even the gorgeous front cover but for the life of me I can't remember the name which is killing me.

Is it maybe Saezuru Tori wa Habatakanai? I haven't read it, but I did a search under the tags mafia, and impotency. Of the 3 that popped up I did read 2 of them, but those two didn't seem to fit.
