Rosario to Vampire and its sequel. The start is mainly a harem though, but later on it focuses more on action.

In some of these, the transformation happens quite late (Apocalypse No Toride for ex) and in others the protagonist is already transformed by the time the story starts but may not know it, but they fit what you want mostly.
