Actual the gender bender part is not the most importand par but the inner conflict (dont know if its the right term for what im after) so something like Inter Sexuality or Hourou Musuko, hope someone understands what i mean...

you want manga where it concentrates on the character's inner conflict concerning their gender.

basara sarasah is troubled over being herself and being tatarah.
after school nightmare The main character is neither fully male nor female and it deals with the psychological aspect of it.
kaze hikaru and kinda this one. She has trouble with her female heart at times.

Well, in a general sense, Family Compo. There's the gag humor element to it still present, but the inner conflict is also there. Gender bender sort of permeates all aspects of the manga, though it isn't always the direct focus.

Kakumei no Hi is really focused on the comedy and the "bishies" but there is an inner conflict. (main character's situation is like that in I.S., but uhhh... post-op, lol)
