There are 2 female love interests, and one of em knows he does it and the other finds out eventually.

I don't think any of these fit all your requirements (some are either lacking an specific element or two, or missing the campy drag queen atmosphere) of, but maybe you'll find something you enjoy anyways?

Definitely has the campy drag queen atmosphere, but I seem to remember it being more gen leaning towards shounenai than het (definitely more of a shoujo feel than a typical shounen ai/yaoi though).

Golden Boy has this one arc where the main character is cross dressing by his own will and a lesbian falls in love with him. This story goes on for about volume 4 chapters 8-12. Also the manga itself isn't about gender bender but only that section.

Maybe Maria Holic. The girl usually hates boys, and only gets all nosebleed-y around him when he is dressed as a girl. I think he is doing it entirely voluntarily, but I don't remember. I saw a couple of episodes of the anime at a convention, and I thought that the comedy came across better in the adaptation.

Usotsuki Lily... most likely already suggested... so I'm seconding. :>

Family Compo maybe? Although the *knowing* crossdressers are one of the main characters parents. (They both crossdress). And it's uncertain whether or not the *daughter* is really a daughter. You'll have to read to see it. Very funny slice of life-ish manga.
