I'm looking for a manga that is lighthearted on the surface but is actually heartbreakingly sad. Stuff like betrayal, unrealized dreams, mind break etc is good as long as the manga has a relatively 'happy' approach/portrayal.
Even a happy manga with just one instance of huge sadness is good too.

Can't See Can't Hear But Love - T___T
Yokohama Kaidashi Kikou - it will give you such a feeling of nostalgia at the end.
Maybe:
Kubera, for me it's one of the rare instances of tragi-comedy in the genre. It tricks you into thinking it's a comedy, then it hits you with the tragedy, and then you realize that even the light-hearted jokes you were laughing at actually hid some pretty horrifying truths. Not sure if it's what you're looking for though.
Dark humor:
Seigi no Mikata - This one's hilarious, the devilish big sister just makes her sister's life a living hell. xD
Sayonara Zetsubou-Sensei - lol, social commentary through satire at its best. xD
Dorohedoro - this one has a pretty unique dark ambience, yet it has a lighthearted tone at the same time.
Edit: Oh hey, how could I forget this! xD
It's Not My Fault That I'm Not Popular! - it has several spin-offs too. It's not sad in the 'heartbreaking sad', but well, it Is pretty sad in the 'wow what a sad loser ahahaha' sense. x'D

Welcome to the NHK! - Ultradark humour about a social recluse. Covers many disturbing topics/themes in a comedic manner.
Maybe Himizu - Probably fitting. I didn't personally care for it because I can't stand this mangaka's style. It looks pretty depressing but it covers many serious topics in a not-so-serious way, until the end.

Take a look at TANAKA Yutaka's Ai-Ren and Itoshi no Kana. He's really good at making you fall out the sweetest and loveliest slice of life straight into the darkest pit, only to pull you up again.
In Wake Up Deadman, Kim Yong-hwan likes to do the same with slapstick humour and sucker punch drama. And the other zombie manga, Sankarea by HATTORI Mitsuru, has its moments, too. (*cough*cannibalism*cough*)
