I Am a Hero's protagonist experiences hallucinations, though they lessen as the series progresses. Very minor romance for most of the series.

If you want to see metaphorically distorted art based on the psychological stress of the characters, Life is Money is a visual treat. The protagonists don't have disorders, but it's cool enough to mention since it's in the same vein.

Himizu and Nijigahara Holograph, but they both have romantic subplots.
