Zankoku na Kami ga Shihaisuru - this one for sure! Drugs appear later on (protagonist will develop a dependence on drugs); manga is quite depressive, but worth reading, I strongly recommend that one!

The first thought that came to my mind was: In The End by Pink Psycho. The protagonist - a rebellious teenager, can be considered as highly depressive and emotionally unstable person. Fits the category perfectly =) (at last in my opinion ).
