I guess it depends on ones definition of a good manga to anime conversion.

Is it staying true and loyal to the plot, characters, and not diverging from the storylines in the manga? (fillers don't count)

Then the BEST are: Naruto, Bleach, Death Note, Hunter X Hunter, Full Metal Alchemist

Is it taking a mediocre manga and making it better? Or taking a great manga and bringing it to life. (these are all titles where i watched the anime before reading the manga. i'm not sure if i would have enjoyed reading the manga as much if i did not have the images of the animated characters in my head before hand.)

Then the BEST are: Samurai Champloo, Hajime No Ippo, Rurouni Kenshin (up to the Kyoto Arc), Hikaru No Go, Full Metal Panic

I try to avoid bad anime titles, but if forced to choose I would have to say the Violinist of Hameln anime adaptation was probably one of the WORST I have seen. GREAT manga, but the anime did not capture any of the same feeling. The OVA did, but that was only one movie.

Best -
Naruto
Bleach
One Piece
Soul Eater
Gintama
Death Note
...And some more that I can't seem to remember right now.

Worst -
Majin Tantei Nougami Neuro (Not horrible, but still pretty bad compared to the original)
Samurai Deeper Kyo (Words cannot express how bad this is.)
...I'm sure there's more but my consciousness apparently erased its badness from my brain.

Best :

Darker than Black
Vision of Escaflowne
Full Metal Alchemist
Inu Yasha
Wolf and Spice

Worst :
Sekirei ( I dunno about others but I felt the voice acting a turn-off )

I will keep updating as I get reminded of others

You sure about this? FMA anime was way different from the FMA manga, with different characters (noticeably the homunculi) and a very different storyline (especially the ending of the anime).

Best (based on material covered these are ongoing): School Rumble, Mushishi, Beck (despite some name changes), Honey and Clover.

Worst: I haven't seen enough manga-anime adaptations to warrant a horrid rating, but the worst I've seen is Naruto, simply because of the horrible, horrible, fillers (which many people undoubtedly watched anyways, myself included).

Best:
Death Note - I loved the anime. The anime honestly helped me understand the manga better. There wasn't much missing.

Worst:
Ichigo 100% - They really killed the anime with the ending. It started out ok, but the ending honestly killed the series. Manga was great!

Does Darker than BLACK even count since the anime came first and then the crappy manga?

Best:

Death Note - For once studios decided to hold off a while before they pounce on their next meal ticket, and it paid off. The story is consistent w/ the manga and the biblical references blend well and add some spice to the show.

Mushishi - The color, the music, the voices, everything matches up perfectly w/ the manga.

Trigun - IDK if I should count this b/c the manga steers in a completely different direction, but that's okay seeing as now we get 2 great stories.

FMA - Same as Trigun.

The Vision of Escaflowne

The Worst:

Gantz - This is the only one I'll give a reason for 1) B/c I'm a Gantz fanatic and 2) B/c it was just too bad. Okay let me say that I loved the show when it started even if it didn't follow the manga 100%, but all of its potential was just tossed out the window in like 3 eps. If you read Gantz after the Buddha Mission you'll find that most of its brilliance lies in its characters and their development. The anime sadly has none of that so it fails.

Zatch Bell, Naruto, and all the anime adapts that just ended w/ little to no conclusion.

Best: Soul Eater, hands down. Bones knows what they're doing. Soul Eater is the perfect alternative to Narutard and Bleah. The story gives equal time to each character without overpowering any, and there is a perfect balance of humor and seriousness. The story arcs also don't take too long to conclude. All these elements are translated perfectly into the anime, however the only problem will be that judging from the pace the anime will end around the
Spoiler (highlight to view)
revival of the Demon God.
Unless there is a second season

Worst: Uhh... the second Gunslinger Girl anime. The character designs and the voice acting completely ruined the mood.
Both of the Negima incarnations were pretty bad too. The first one stayed truest to the story but the lame attempts at fanservice with the sub-par animation made it the least appealing. The second one not only went off into left field, but a lot of the scenes made me wonder what medications the staff was on when they made it. It really got, shall we say, SHAFTed.

Bad ones: Naruto, Inuyasha, R kenshin (the last season++. NOT the OVAs)

biggrin cool

Definitely the best manga-anime adaptation IMO would be
Black Lagoon
Slam Dunk - aside from the fact that it was discontinued, it was pretty good
School Rumble
Full Metal Alchemist - Same reason as Criminal Intent
Hunter x Hunter - aside from the fact that it was discontinued, it was pretty good
Claymore - Aside from the ending, I thought it was pretty good compared to the manga artwise
Hajime no Ippo
Basilisk
Eureka Seven
Death Note
Lovely Complex - love the Kansai Dialect
Soul Eater
Ouran High School Host Club
Kuroshitsuji - is looking good so far
RAVE - aside from the fact that it was discontinued, it was pretty good

As for worst, same as everyone else who says Naruto, Bleach, Inuyasha, Ichigo 100%, Tenjo Tenge, Rosario+Vampire and alot of other ecchi series

for me "best" doesn't mean following manga word by word, the main thing is to make all characters move and speak the way you can recognise and love them. the second thing is of course atmosphere.
so, the "best"
Hikaru no Go
Death Note
Genshiken
Zetsubou Sensei (though I read only first volume)

really difficult to pick up the "worst"...
Gantz
Hellsing (though music was awesome, everything except it was...)

As for the best, i think many have been mentioned...
but for the worst i have to go for: Eyeshield 21, even tough it could be adjusted to the original plot, the execution sucked @ss and sucked bad... Worst seiyuus ever, an OST that can't stand on its own, and the lack of an almost nice animation...

I'm inclined to say that the "best" adaptations are the ones that have more added to them through the music and extended visuals- shows like Mushishi or Aria that are already heavily atmospheric to being with.

Other than that, as far as adaptations go I prefer things to be as close to the manga as possible, although there are one or two exceptions that prove the rule. So, Monster basically.

@Monstar Maalik: Kure-nai, FMP, and Toradora are all adapted from light novels, not manga. Not that it matters much, but unless you've read the source material, it's difficult to judge their merit as an adaptation. [/worthless technicality]

I'll be silent on the worst adaptation front, I think, because nothing has horribly offended me or anything, although I was a bit disappointed with the anime adaptation of Planetes, relative to the manga, anyway.
