I'm trying real hard to come up with an example here.
Erotic Horror which is a collection of oneshots. Some oneshots by Shinohara Chie and Kaori Yuki.
Like... the main character falls in love with someone, but at the end we discover that their lover never existed, it was all their own fantasy. Or a psychic predicted that the main character will kill their lover and they ended the relationship out of fear, but the ending revealed that the psychic himself is in love with the main character and did it to break them up. A widow kills herself after her husband dies, but at the end we discover that she faked her death because she wanted a new life with her lover and the money they stole from her dead husband. This kind of stuff.

Sounds like one of my favorite oneshots called Black Peach Flower

Maybe the oneshot in Conflict Lover. The twist isn't as extreme as your examples, but it's there and the relationship is fairly creepy.

In Shuuseki Kairo no Himawari, The Other Side of the Rose Wire (very disturbing) and Alive. Some of the others may have twists too but I can't remember.

Chapter 5 of Knock Your Heart Out! (Maki) has a twist but you find out the truth mid-chapter; the story is more about her dealing with the twist than the twist being used as a shock device. It's not a disturbing story either.

I second Black Peach Flower, the art in that one is so innocent too...

probably not exactly what your looking for but i did not expect the ending to Melodramatic Library at all.

and its really short but maybe also Wasure Yuki (AMANO Shinobu) -its not really a romance though.

edit: OH! i thought of one that fits perfectly! Hal (AYASE Umi), it also has an anime if you perfer that. but yeah i never would have guessed the ending of this one.

Sorry, I don't particularly have any recommendation, but I just have a question on that psychic person story. The way you described that story is certainly not what I read in Erotic Horror (2nd) story. I reread the story again to make sure too, or are you trying to make an "ideal" example type of thing? Because personally I think that would be cool, yandere potential.

"Aishiteru", Uso Dakedo. - It's kinda brushed aside in the story, but the guy did something I never thought I would see in a shoujo manga.


+1 Risou no Kareshitachi - It starts out so normally too!

Atashi wa Sore o Gaman Dekinai - Story 2 is where it's at! Can't say anything else without spoiling it.

Maybe Melo Holic and All you need is Kill?

Some early arcs of Franken Fran also have such 'romance' plot twists.

oh yeah i totally forgot about Risou no Kareshitachi! this one is crazy!

and i thought of another one, the first story of Yumenara Samenaide. this ending was a little more predictable but still very interesting.
