I don't see Homunculus anywhere. The theme may be realistic (or not, depends on how you view future medicine and stuff bigrazz) but the drawing style is really realistic. And, hm, Sanctuary, maybe?

art-wise: berserk, sanctuary, and vagabond.

story wise: beck, genshiken, and vagabond i guess. it is more or less a biography of miyamoto musashi...

Doesn't the OP want a Romance/Shoujo kind of manga? o.O

Ichi the Killer, Battle Royale, Gantz, Tenjou Tenge,... and plenty of others that were suggested here aren't exactly that. xD Though they're all good in their own way.

Toshiue no Hito is realistic (if such small women do exist..)
Didn't really understand what you are looking for, but I think this is realistic enough~.~
Looks like a lolicon though...

If it's sth with really realistic art, then go for Sanctuary and Crying Freeman. They're nothing like shoujo manga, though you can find a decent dose of romance there. The art is extremely realistic.
Maybe also Cat Street and Dolls

As for the shoujo manga w/o bishies...there is none? The guys simply are handsome. Period.

If you'd like to see at least well-drawn bodies, try some Kayono's works ( she must have read some anatomy books...but it looks weird anyway ). The guys are extreme bishies anyway...

Well if u dont care that its not shoujo then...
Air Gear
Gantz
Tenjou Tenge
And by realistic i mean realistic by art

If it's realistic by art, then you gotta read Battle Royale and Ichi the Killer.

I think you guys aren't getting the point. He/she is looking for more of the slice of life type of mangas that touches everyday issues.
Seriously did anyone even check the reference title Living game

Akkan Baby Somewhat unrealistic, but it's a shojou without bishounens
Dance till tomorrow It's got the same feeling as Living game imo
[m]issho asonde kurashitai[/m] I liked it because the female lead wasn't a pushover
and was realistic enough to have her own mind

There are shoujos without boygirls ;) Living game is one of them, I""s, also one someone recommended in this thread, Maison Ikkoku, ;)

Anyways thanks all, got a lot of stuff to download.

Yup, Maison Ikkoku is pretty awesome. However, to the person above me: Maison Ikkoku and I"s aren't shoujo (the first is seinen, the second, shounen).

In terms of shoujo/romance... I think Lovely Complex is fairly realistic. The love interest is pretty far from being perfect at all, and there are, if I recall correctly, two "pretty boys" in the series entirely - one of whom is used for very little besides comic relief. Great manga.

Nana does have fairly good-looking men but the romance and character interactions are realistic enough that the mood of bishounen manga isn't present.

Ah well then I guess I wasn't really looking for shoujo, but more romance kind of mangas? Still not very familiar with all the terms. haha .NaNa seems good, going to try that out too.

Second Bitter Virgin. Can't really get anymore realistic than that.

You can try Pastel, and other works by the same author (youll have to look him up) also strain, G, Heat are great. Operation Liberation of Men, Endo's short stories, Freesia, Blue Heaven. There is also another one involving a cruise ship in which two people are found over board and people start dying, forgot the name tho. Try Fuan no tane (horror)

Hajime no Ippo's pretty realistic. No superhuman strength either.

Jojo's Bizarre Adventure is also pretty realistically drawn but in that one there are superpowers.
