For age gap Junjou Romantica is a must read. It's also yaoi but not that hard core. Most of the pairings have a large age gap. Most of them.
Loneliness...Pretear was really good for that. I only watched the anime so i don't know about the manga. But manga is usually better than the anime so I don't think you'll have any problems.
For guardian realtionship...Kuroshitsuji. If you don't mind the creepiness, demons, and gore-ish. It's same gender but no yaoi or shounen ai. It's also kinda depressing.
If you don't mind hard-core then Hoshi no Yakata is a yaoi prostitute manga. It's also S&M if that doesn't bother you. A sorta prostitute on is Love Celeb. I think...it's also show biz. I haven't finished it so I can't tell you anything else, sorry.

Tsubasa Wo Motsu Mono: I' m pretty sure the guy is older than her (She's 16, they naver give an age for him, but he seems older)
Guardian Relationship:
RG Veda, kinda....
Cynical Orange, maybe? And it can also kinda fit into the school life drama
Her older step-cousin likes her, but she falls for another guy and the climax of the story is fairly dramatic
School Life Drama:
Natsume Yuujinchou
It's a rally nice story about a lonely boy that can see ghosts..(It's a lot better than the lame summary I just gave)

the actual age gap isn't mentioned but it's pretty wide if you think about it.
