Honey & Clover - The gold standard of mature romance/slice of life. Fantastic anime adaptation, also adapted to a live action movie along with live action TV Series in both Japan and Taiwan.

Horimiya - My favorite ongoing romance right now, great comedy, great story, no love polygons

Kimi ni Todoke
Cross Game
Natsu no Zenjitsu

I'll make this short:
my lists
take the high rating ones and ignore if (the unusual case occurs that) there is no romance tag.

I'm surprised to see such a small amount of replies. Where did all the drama-loving manga fans vanish to?

YAMASHITA Tomomi's Apothecarius Argentum
Romance of the forbidden kind with plagued characters and highly dramatic plot twists.

Full Metal Panic! Σ by GATOU Shouji/ Shikidouji and UEDA Hiroshi
Mecha with school life elements and great characters; plot and setting are not to be underestimated! It has already an anime adaption (2 seasons + a spin-off) for a prequel, and I'm sure it would make a great comedic action drama.

HATTORI Mitsuru's Sankarea
Another one with an anime adaption. Stars off as a light-hearted romance comedy with an unusual premise scratching some uncomfortable issues every now and then, and before you know it the whole story has developed into a gripping drama with more action than you would have ever imagined. Anyway, it is a nicely twisted story.

Reimei no Arcana by TOMA Rei
A typical premise (arranged marriage between enemies) soon turns into an inner conflict about personal feelings and royal duty.

The One by LEE Nicky
Moderately suspenseful at the beginning but picks up the pace after a while. Rest assured, peaceful moments will always be followed by (un)forseen plot points.

RYAN's Ingènuo
Probably the most dramatic age gap romance. It will have taken your heart across the globe before they finally get together.

Suzuka by SEO Kouji
Almost a classic which would make a good script for a soap opera. Still enjoyable, though if you like Shounen rom-coms with a tsundere heroine and your beloved harem. Forst part has been turned into an anime series.

GOTOU Junpei's Ginen Shounen
Nicely drawn-out story about the struggles of a photography-loving boy who suddenly gains unwanted precognitive skills through his camera lense.

Ayashi no Ceres by WATASE Yuu
The vengeful spirits of two former spouses reincarnate into the bodies of two twin siblings and turn their peaceful everyday life into a never-ending alternation of hunter and hunted. Follow the twins' struggle to escape their cruel fate. Has been adapted to anime.

KONAMI Shouko's Shinobi Life
What do you do when a ninja suddenly enters your life and calls you "Hime"? Excessive time travelling, you have been warned.

Boku no Hatsukoi o Kimi ni Sasagu by AOKI Kotomi
Already adapted to a live action drama. Perfect example of a tragic love hindered by many outer obstacles. The main characters' love for each other is made crystal-clear from the very beginning, but after heaps of unwanted miscommunication, bad timing and purposely created misunderstandings things don't seem to be this easy anymore. Great material for a TV drama.

TAKANASHI Mitsuba's Akuma de Sourou
Another one with a life action adaption. Secret romance between step-siblings who live under the same roof. Add jealous school mates, bad rumors and a painful past among other problems and you'll get this interesting series which manages to keep most of its twists and turns within acceptable limits.

Shingetsutan Tsukihime by Type-Moon/ SASAKI Shounen
How do you become your beloved vampire princess' love-interest? By cutting her up on first sight, of course! Dating with regular monster extermination, lovey-dovey everyday dates, blood, and more cutting and breaking - such a description would fit too well if it weren't for the plot's seriousness. Based on a visual novel.

MORI Kaoru's Emma
Realistic victorian romance between a noble and a maid. Lots of social hardship against a vivid historical setting. Adapted to anime.

Molester Man by YOKO
Heartfelt and true story about a closet-otaku living through a period of transition. Friendship, love, character growth – it all starts with a molesting incident.

MOTOMI Kyousuke's Dengeki Daisy
Not to be described with words. Read it if you haven't done it already.

Mars by SOURYO Fuyumi
Like David Mamet pointed out once, THE SCENE MUST BE DRAMATIC. Not much comedy but definately enough material for a good TV show.

OGAWA Yayoi's Kimi wa Pet
Contemporary love triangle (and more) between an office lady, a collegue and her human pet. Interesting predicament for various reasons. Adapted to live action.

Guru Guru Pon-chan by IKEZAWA Satomi
Comedy paired with unexpected drama - how else would you tell the love story between a human boy and a female dog?

MATSUMOTO Tomo's Kiss
Beautiful couple growth surrounded by well-chosen musical motifs. Here's a link to all the music pieces involved: KISS (Matsumoto Tomo) - Music from the manga

Nodame Cantabile by NINOMIYA Tomoko
Slice of life against a classical musical background. Humourous and well-developed couple growth. Has its own anime.

Seconding Natsu no Zenjitsu.
