Banana Fish is kind of long, but it's the saddest manga I've ever read.

I've only read a few manga that I would classify as "tearjerkers", but whether they come off as "so sad it hurts", "so sad I'm crying", or "this just makes me angry" is up to you. They all have varying levels of sadness, so don't be fooled if one starts slow or light.

Not Simple is only a volume long, but is one of the most depressing stories I've ever read.

I second Banana Fish. It is the best manga I've ever read, and I've read a lot in the last 10 years. It is long, but doesn't drag at all and if it doesn't have you in tears by the end, I question if you have working tear ducts because it is heartbreaking.

I fourth Watashitachi no Shiawase na Jikan, it lacks the same emotional impact as Banana Fish, mostly because it is so short that it becomes hard to develop the same connection with the characters, but it is the best short tragedy I've come across.

Vampire Juujikai is good, and has a bittersweet ending, but not as realistic as the other two.

Short manga (less than 10 volumes)
Akuma no Ororon - Is a sad cross-species love story which is a lot more gritty than traditional romance. (I wouldn't even call it a romance)
ES is a psychological drama about a man with ESP powers.
New York New York tells the tale of two gay men in, you guessed it, New York. It is very sweet, but also deals with very real and difficult issues like homophobia and HIV.
Cut - 2 emotionally damaged high school boys meet and find solace in each other. The ending isn't so sad, but the background of the lads is awful and the whole manga has a very brooding tone to it.
Lovers, souls _ So this is one of a series of interconnecting works in Miyamoto Kano's Rules universe. However, it is the first time the readers meet Tooru, who is a main character in Rules, so this can be read as a stand alone (I can almost guarantee that you'll be come hooked and want to read more though). Her earlier work, Hydra, which belongs to the same universe also has a tragic character and a generally angsty overtone. In fact, pretty much everything by Miyamoto Kano oozes angst, drama, and unfulfilled wishes with a cast of tragic backstories and emotional scars.

Longer manga (10+ volumes)
Let Dai is similar to Banana Fish, but much more poetic in its imagery and without the complex subplot to drive it onwards.
Zankoku na kami ga shihaisuru is a very harrowing and realistic portrayal of the realities of sustained sexual abuse.
Sanctuary is a slightly over-dramatised, but very touching story of 2 young men and their journey to change Japan.
